Benefit Change Request Summary Report
Purpose
This document outlines the process of how to view change request reports within the Green Leaf Elevated System.
How to view Benefit Change Requests
Log into your Elevated account and navigate from your menu to the Team Tab > Benefits > Enrollment Requests > Change Requests Summary.
This report will show you any benefit change requests that have been submitted by employees.
Column Headers
- Employee ID - Shows Employee's ID number.
- First Name/Last Name - Shows Employee's name.
- Employee EIN - Shows the company EIN for this change request.
- Employee Status - Shows Employee's current status.
- Approver First Name/Last Name - If the request has been approved, shows Approver's name.
- Effective From - Shows the Effective Date of the change request.
- Status - Shows the status of the change request(Open, Submitted, etc.).
- Workflow Status - Shows the status of where the change request is on the Workflow.
- Type - Shows the Type of the change request, such as a New Employee.
- Life Change Event Type - Shows the Life Change Event of the change request, if applicable.
- Request Benefit Profile - Shows the Benefit Profile that the change request is attached to.
- Date Submitted - Shows the date in which the change request was submitted.
- Date Completed - Shows the date in which the change request was completed.
- Created - Shows the date in which the change request was created.
- View Selected - Allows you to view the selected Benefit Change Request.
Delete - Allows you to Delete the selected Benefit Change Request.
*This action is irreversible; please use caution when clicking this icon.
- View Account Benefit Plans - Clicking this will navigate to the Employee Benefit Plans page, where you can view all employee benefit plans in the system.
- View Change Requests Detail - Clicking this will navigate to the Benefits Change Requests Detail page, which shows a more detailed and comprehensive look at each change request currently in the system.
