Custom Form Reporting

Purpose

This document outlines the process of how to view custom forms assigned to employees within the Green Leaf Elevated System.

How to View Assigned Custom Forms

Log into your Elevated account and navigate from your menu to the Team Tab > HR > Forms > Other Forms

This report will show you all Available, Open, and Submitted forms in the system.

Available - Shows available forms within the company that can be assigned to an employee.
Open - Shows open forms that are currently assigned to employee(s) but have not been submitted or completed.
Submitted - Shows forms that are assigned to employee(s) that have been submitted, verified, or completed.

You can navigate to each section by clicking on the tab name referenced in the screenshot below.



Available Tab

Functional Buttons

  1. Preview Icon - Allows you to preview the form
  2. View Open - Allows you to see open (non-completed) forms that are assigned to employees
  3. Assign - Allows you to assign this form to an employee



Open Tab

Column Headers

  1. Employee - Displays the name of the employee that the form is assigned to
  2. Employee Status - Displays the status of the employee in the system
  3. Form - Displays the form name that is assigned to the employee
  4. Created On - Displays the date that the form was assigned to the employee
  5. Status - Shows the status of the form that is assigned to the employee
  6. Supporting Documents - Shows if any supporting documents were attached to this form and allows you to preview them
  7. Notes - Displays any notes that were attached to this form
  8. Actions - Displays an ellipse icon that presents the following options when selected:
    1. Add Notes - Allows you to add notes to this form that is assigned to the employee
    2. Edit - Allows you to edit a form that is assigned to the employee
    3. Upload Document - Allows you to upload a supporting document to the form that is assigned to the employee
    4. Assign Another - Allows you to assign this form to another employee
    5. Download PDF - Allows you to download the form as a PDF
    6. Delete - Allows you to delete the form that is assigned to an employee
    7. View Detail Report - Allows you to see a more detailed report on the form that is assigned to an employee
    8. View History - Shows the history for a form if it has been assigned to an employee more than once

Functional Buttons

  1. Checkbox - Allows you to select a form for an employee or group of employees
  2. Download - Allows you to download selected forms as a PDF
  3. All Employees - Allows you to filter the report down to look for a specific employee or group of employees



Submitted Tab

Column Headers

  1. Employee - Displays the name of the employee that the form is assigned to
  2. Employee Status - Displays the status of the employee in the system
  3. Created On - Displays the date that the form was assigned to the employee
  4. Status - Shows the status of the form that is assigned to the employee
  5. Supporting Documents - Shows if any supporting documents were attached to this form and allows you to preview them
  6. Notes - Displays any notes that were attached to this form
  7. Actions - Displays an ellipse icon that presents the following options when selected:
    1. View Details - Allows you to view the submitted form for an employee
    2. Upload Document - Allows you to upload a supporting document to the form that is assigned to the employee
    3. Assign Another - Allows you to assign this form to another employee
    4. Download PDF - Allows you to download the form as a PDF
    5. Delete - Allows you to delete the form that is assigned to an employee
    6. View Workflow - Allows you to view the workflow associated with the form

Functional Buttons

  1. Checkbox - Allows you to select a form for an employee or group of employees
  2. Download - Allows you to download selected forms as a PDF
  3. All Employees - Allows you to filter the report to look for a specific employee or group of employees
  4. All Statuses - Allows you to filter the report to look for forms with a specific status
  5. This Year - Allows you to filter the report to look for forms assigned in a specific year or all years
