1094-C Reporting

Purpose

This document outlines the process of how to view 1094-C forms within the Green Leaf Elevated System.

How to View 1094-C Forms

Log into your Elevated account and navigate from your menu to the Team Tab > HR > ACA > Forms > Employee Forms 1094-C
This report will show you all of the 1094-C forms that have been documented for your employees.


Column Headers

  1. Company EIN - Shows the EIN of the company.
  2. Company Name - Shows the name of the company.
  3. Line 18 Count - Shows the total amount of 1095-C forms that will be submitted with the transmittal of the 1094-C.
  4. Last Modified - Shows the date when the 1094-C was last modified in the system.
  5. Finalized - Shows if the 1094-C for the selected year is finalized.
  6. Created - Shows the date when the 1094-C was created.

Functional Buttons

  1. Pencil Icon - Allows you to view, edit, and/or finalize/unfinalize the selected 1095-C form.
  2. Trash Can Icon - Completely deletes the 1095-C form from the employee's records. *This action is irreversible; please use caution when clicking this icon.
  3. Selected Year - Controls which year the 1094-C forms currently displayed correspond to. Select this to change which year you would like to view.
  4. Add New - Adds a new 1094-C for the selected company EIN.
  5. View Selected - Allows you to view the 1094-C forms you have selected from the list.
